|04/12/2012||Area Air and Sub-Slab Air Quarterly Monitoring Report – Building 4.||Contaminant migrations may exist through perforations in the building foundation slab. However, based on identified contaminant concentrations in the indoor air and crawlspace samples collected, it does not appear that contaminants are migrating to employee-occupied spaces at concentrations above the applicable site-specific screening levels.|

Services Administration (GSA) at the Bannister Federal Complex (BFC), specifically Building 4,
located at 1500 East Bannister Road, Kansas City, Missouri.
|Contaminant migrations may exist through perforations in the building foundation slab.
However, based on identified contaminant concentrations identified in the indoor air and crawlspace samples collected, it does not appear that contaminants are migrating to the employee-occupied spaces located above the crawlspace at concentrations above the Site-Specific Screening Levels.

This sampling activity was the first of four planned sampling events.|
|09/26/2011||Building 4 Crawlspace Limited Removal Activities Report.||Laboratory data was reported within acceptable ranges for relevant quality control parameters (eg.
laboratory control samples, matrix spikes, etc.) and no results were rejected through Columbia
Analytical Services and Test America quality control reviews. Internal laboratory QA/QC packages are provided with the Appendix B analytical reports.

|06/17/2011||This document pertains to Building 28, as well as Building 4. It contains a report from the General Services Administration on limited removal from Building 4 crawl space and the Building 28 sump pump.||Limited removal of arsenic, lead and asbestos debris. Subsequent air testing clear.|
|01/07/2011||This document is the results of an Indoor Air Quality Assessment performed as part of ongoing safety and environmental management plan.||CO2 readings indicate need for additional fresh air and stained ceiling tile was removed. HVAC was adjusted for fresh air.|

|04/08/2009||This document contains the results of Periodic Groundwater Sampling Buildings 4 and 50 at Bannister. The Environmental Protection Agency review determined that additional periodic Groundwater sampling was required.||Environmental Protection Agency reviewed the PA/SI report, accepted the results but suggested some additional, limited investigation.|
